In all six cases, the root was enlarged (37-54, mean 43 mm). However, in only two were the aortic wall echoes thought to be abnormal. One patient was thought to have an ejection sound and was therefore labelled as bicuspid with root dilatation. Conversely, four patients with non-inflammatory root disease were thought to have thick aortic walls and were wrongly called inflammatory. It is obvious that the sensitivity and specificity of this sign are too low to be useful. Idiopathic aortopathy The characteristic echocardiographic features of this condition are an increased root diameter, often with multiple echoes arising from the rolled cusp edges, but with no ejection sound and a normal mitral valve (Figure 7).
